1)    Copy of the most recent amended complaint (lawsuit) (filed or draft)

2)    Medical records documenting Kugel Mesh surgeries, bowel perforations,
  chronic enteric fistulas, infections, strokes, heart attacks, or death.  

       And any pre-existing conditions.

3)    Expert reports (if any).

4)    Settlement Agreement (if any)
 

5)    "Net Letter" from the attorney's office, indicating the gross settlement
   amount and the net amount to the client after: attorney fees, expenses,
   and any liens are paid.  And the approximate timing of when the
   settlement proceeds will disbursed.
