In addition to calling your attorney, we will fax and email the
below request to your attorney, so they have something in clear writing
letting them know what we need to review on your fen-phen claim.
And please note: You can always send us your paperwork directly, as opposed to us obtaining it from your attorney. It is often much faster to get the requested paperwork from you versus your attorney.

client, (Fen-Phen Claimant's Name Here), has contacted our firm requesting an advance on her
pending fen-phen claim. Please fax the following documents (if they exist) to 858-244-4977: 1)
Paperwork from
the Fund Administrator describing qualified benefits and Relative Payment
Value, etc. 2)
Settlement Letter
/ Settlement Offer 3)
Net letter from
attorney indicating gross settlement and net amount to client after attorney fees,
expenses and liens are paid. Please
include whether the client has already received her initial 40% disbursement
and if yes, the approximate timing for the remaining 60% disbursement, and if any
prior advances/liens have been paid off. Please contact me with any
