Your client, (Your Name Will Go Here), has contacted our firm requesting funding on his / her pending lawsuit against Bausch & Lomb. 

Please fax the following documents (if they exist) to 858-408-3236:

1)    Copy of the most recent amended complaint (lawsuit) (filed or draft)

2)    Medical records documenting eye problems, i.e. relevant surgeries
  ER trips, eye fungal infections, cornea scarring, cornea transplant, or

       blindness.   

3)    Expert reports (if any).

4)    Settlement Agreement (if any)
 

5)    "Net Letter" from the attorney's office, indicating the gross settlement
   amount and the net amount to the client after: attorney fees, expenses,
   and any liens are paid.  And the approximate timing of when the
   settlement proceeds will disbursed. (Note this item will only be available

        a settlement agreement has been reached.)
