Please fax the following documents (if they exist) to 858-244-4977:

1)    Copy of the most recent amended complaint (file lawsuit) or copy of the

       Tolling Agreement

2)    Copy of the Retainer Fee Agreement

3)    Prescription history records showing you had been prescribed Avandia

       sometime in the past.

4)    Medical records documenting Avandia use and damages such as
  heart attack, heart failure, stroke, bone fractures, or death.  And any  
  pre-existing conditions.

5)    Expert reports (if any).

6)    Settlement Agreement (if any)
 

7)    "Net Letter" from the attorney's office, indicating the gross settlement
   amount and the net amount to the client after: attorney fees, expenses,
   and any liens are paid.  And the approximate timing of when the
   settlement proceeds will disbursed.
